Use Elgin's application and affidavit

The request includes an affidavit about recent course use, license and insurance proof, a plea, and the applicable court payment. The city explicitly says to wait for permission before enrolling.

Return the record and certificate together

Elgin describes a 90-day deferral after approval for the completion certificate and a certified Type 3A driving record. Its court office is at the City Hall Annex on North Main Street. The website advises calling ahead because staff may be unavailable during hearings or training.
