Use the sworn request before taking a course
Frisco accepts requests online, by mail or in person. Complete the sworn request with the required license, insurance and court payment. Its page explains that multiple eligible offenses on the same citation may be addressed with one course, but court charges apply separately to each offense. Ask the court which charges are included in your approval.
Submission is a separate task
After approval, return the signed completion certificate and certified Type 3A record within the assigned 90 days. Frisco lists mail, in-person delivery and an online DSC Completion Submittal Form. The request form and the completion form serve different purposes; save a receipt for each stage. Its published process does not offer routine extensions.
