The official page provides a plea sheet and payment-plan forms. Ask the criminal clerk which course application and attachments are needed before treating a plea sheet as a complete request.

Bright Driving Safety is TDLR Course Provider #CP1265. The online course is $25; court costs and driving-record fees are separate. Submit your certificate and all documents required by your court on time.
