Read the affidavit before signing
The JP1-1 form requests license and insurance evidence and a sworn signature. Confirm the current court costs before payment. Its stated 90-day period runs from notarization, so coordinate signing with the court’s approval process.
Return both completion items
The affidavit calls for the certificate and driving record and says the certificate must name JP1, Place 1. It lists delivery in person, by mail or to jp1-1@hayscountytx.gov; retain confirmation that both items arrived.
