The form asks for license and insurance copies, a notarized signature and a cashier’s check or money order. It lists $146; verify that amount with JP2. The forms page directs completed paperwork to jp2@co.hill.tx.us.
Confirm receipt and the final due date
The form says compliant applicants may receive no further correspondence and gives 90 days from the cited appearance date. Confirm receipt and authorization directly before taking the course, and ask when the Type 3A record and certificate must arrive.
