A written request can be emailed
Huntsville lists mcourt@huntsvilletx.gov for written requests, alongside mail and in-person delivery. Include the request affidavit, license, insurance and court costs. The form identifies acceptable mailed payments; personal checks are not accepted.
Return one complete packet
Huntsville asks for the signed court certificate and certified Type 3A record together. Avoid sending only the course certificate and assuming the record will follow automatically.
