Match the affidavit to the violation
Irving’s forms page separates school-zone and non-school-zone requests. The request requires a plea, valid license, insurance, affidavit and nonrefundable court costs before the course. Its instructions describe online and in-person requests and delivery of the printed affidavit by email, mail or eFax.
Allow time for the reply and the return packet
If applying by mail, wait for written approval. Irving says faxed requests receive replies by mail or email rather than return fax. Once approved, complete the course and return the signed certificate with the certified Type 3A record within the assigned 90-day period.
Use the mailing address for mailed papers
The physical court is at 305 N O’Connor Road. Mailed correspondence goes to PO Box 152288, Irving, TX 75015-2288. Use the current court page for contact details rather than an older saved affidavit.
