The published list includes the signed reply form, liability-insurance proof and $144 in court costs, or $169 for a school-zone offense. Return the request by the appearance date and confirm the judge’s approval.
Set a separate reminder for the driving record
Because the court permits the Type 3A record to follow the application, ask for its exact return date. Keep that requirement beside the course certificate deadline so neither item is overlooked.
