Complete both pages of the request
Killeen’s application includes a plea and eligibility affidavit on page one and a separate information sheet on page two. It asks for a valid license or qualifying military documentation and insurance listing you as a driver. Both pages contain sworn-signature areas; follow the notary or deputy-clerk instructions. Select the correct course type rather than using a standard course for a specialized violation.
Keep identification with correspondence
The court’s portal supports driving-safety requests for eligible cases. Court email instructions ask for photo ID, your full name and citation number. Confirm the final return checklist and deadline with the clerk at 254-501-7850; they are not stated in the request PDF.
Plan a counter visit
The court lists Monday–Thursday counter hours and Friday closure. Use the current court page to check hours before traveling to 200 E Avenue D, Suite 1.
