Use the July 2026 checklist
The driving-safety section asks for insurance evidence, a certified Type 3A record and a money order. It lists $144 for moving violations or $169 in a school zone; confirm your total with 432-688-4722.

The form directs all pages to JP2 at 301 South Main Street, second floor. This differs from the Washington Street address printed on an older shared JP2/JP4 form.
