Request at the cashier window or by mail

Midland permits a request at the court’s cashier window without first seeing the judge, or by mail using its request form. Eligible cases may also have an online DSC option. Do not confuse that option with paying the ordinary fine; contact the driving-safety office at 432-685-7315 if unsure.

Include every page of the record

The court requires a certified Type 3A record and describes the complete document as ending with “end of record.” A different record type or incomplete printout does not meet its instructions. Return it with the official course certificate within the assigned 90-day period.

Telephone questions do not meet the appearance requirement

Call to clarify paperwork, but Midland explicitly says an appearance cannot be entered by phone. Make the required filing or visit by the 14-day deadline.

Official instructions and forms

Check the linked court material for changes before filing. This is a Bright Driving Safety guide, not an official court website or an individual eligibility decision.
