Prepare the record before a mailed request
JP4’s driving-safety page requires a notarized request, certified Type 3A record, insurance and license copies, and the specified court payment. It says online requests are not accepted. The page lists $145 by cashier’s check or money order payable to Montgomery County with the cause number.
Use the current address, not the old PDF footer
The current court website gives 21130 Legion Road, Suite 100, New Caney, TX 77357. An older linked requirements form still carries Justice Drive. Confirm delivery details at 281-577-8970 before mailing papers copied from that older form.
Return the signed completion evidence
The page directs drivers to present the signed court certificate and affidavit by the 90th day. It lists in-person, mail, fax or email delivery for completion papers, even though initial requests must be in person or by mail. Use the precise due date in your approval.
