Use the January 2026 request form
JP5 publishes a precinct-specific driving-safety or motorcycle-course form. Complete, sign and date it, attach the required copies and proof of insurance, and include the stated court payment. For a mailed request, add a self-addressed stamped envelope so the court can return its response.
Follow JP5’s payment instructions
The form lists $145 payable to JP5 and allows the specified check, money order or cashier’s check; it excludes temporary and out-of-state checks. Confirm the amount with the clerk if your case has different circumstances.
Get the completion terms before enrolling
The form does not itself set a completion period. Ask JP5 for the approval, exact due date, driving-record requirement and how to deliver the certificate. Defendants under 17 must appear with a parent or guardian under the form’s instructions. This office is on Unity Park Drive, separate from Magnolia Municipal Court.
