Pasadena keeps different forms for offenses before September 2019, late 2019, and January 2020 onward. The current form requires the application and declaration, license copy, insurance identifying you, and court payment. It permits a counter request or certified mail with return receipt requested to PO Box 1575, Pasadena, TX 77501-1575.
