Apply with the precinct’s form
JP4’s form includes a no-contest plea, calls for a money order and insurance with the request, and directs non-speeding cases to the court. Verify the current court charge before sending payment; the form’s printed amount should not substitute for the clerk’s instructions.
Check the return date and Friday hours
The Highway 42 office lists Monday–Thursday hours of 8:30 a.m.–4:30 p.m. and Friday hours of 8:30 a.m.–noon. Ask the clerk for the approved completion date and which documents must accompany your signed certificate. The request form itself does not establish a 90-day clock.
